  5.     To my family, my physician, my lawyer, my clergyman.

  7. To any medical facility in whose care I happen to be.  To any
  8.  
  9. individual who may become responsible for my health, welfare
  10.  
  11. or affairs.
  12.  
  13.     Death is as much a reality as birth, growth, maturity
  14.  
  15. and old age -- it is the one certainty of life.  If the time comes
  16.  
  17. when I, #NAME#, can no longer take part
  18.  
  19. in decisions of my own future, let this statement stand as an 
  20.  
  21. expression of my wishes while I am still of sound mind.
  22.  
  23.     If the situation should arise in which I am in terminal
  24.  
  25. state and there is no reasonable expectation of my recovery, I
  26.  
  27. direct that I be allowed to die a natural death and that my life
  28.  
  29. not be prolonged by extraordinary measures.  I do, however, ask
  30.  
  31. that medication be mercifully administered to me to alleviate
  32.  
  33. suffering even though this may shorten my remaining life.
  34.  
  35.     This statement is made after careful consideration and
  36.  
  37. is in accordance with my strong convictions and beliefs.  I want
  38.  
  39. the wishes and directions here expressed carried out to the 
  40.  
  41. extent permitted by law.  Insofar as they are not legally enforce-
  42.  
  43. able, I hope that those to whom this will is addressed will
  44.  
  45. regard themselves as morally bound by these provisions.
